Reporting Misconduct
Anyone in the Diocese of Ontario is free to make a report to the canon pastor whether they are a complainant, respondent or a witness to any misconduct.
A report should be made with regards to any misconduct that involves any form of abuse.
Abuse is usually an ongoing pattern of behaviour but can also be a single incident. It happens when a person takes advantage of their power in a relationship or situation to say or do things which:
- cause harm to the other person, or
- intend to cause harm to the other person, or
- put the other person at risk of harm.
Abuse can include, but is not limited to:
- bullying
- cyber abuse (abuse via social media, email, text messaging, online gaming and so on)
- emotional abuse
- financial abuse
- harassment
- neglect
- physical abuse
- sexual abuse
- spiritual abuse
- concealing the abuse of another person.
How to report:
Use the following fillable form to download and complete a written misconduct report. Once completed, sign and then scan a copy to send to the canon pastor.

Email the canon pastor, the Rev. Canon Margaret Johnston-Jones: canonpastor@ontario.anglican.ca.
Once you have reported an incident:
- The canon pastor will contact the person who is making the report within 48 hours of their making the report to obtain as many details as possible about the incident.
- The canon pastor will contact all involved to arrange individual, confidential interviews.
- Interviews will be scheduled within one week of receiving a report. Interviews will preferably be in person at a location deemed safe by both the interviewee and canon pastor, or via Zoom if necessary.
- Meetings between the canon pastor and the parish churchwardens and incumbent will take place with the written consent of the complainant.
- The bishop of the Diocese of Ontario will receive a copy of the misconduct report regarding the details as well as a plan of action for a resolution.
